A current copy of this form must be filed with SDAR to activate or ensure uninterrupted signing and charging privileges. This form must be filed or renewed at the beginning of every year or an interruption in service will occur.

Authorized Auto-Debit Form

To authorize SDAR to automatically deduct your payment for dues and services from your checking account or credit card, fill out the Auto-Debit Form completely and fax to (858) 715-8088.
