Agent REALTOR® Application Form and MLS Application - This form includes the SDAR REALTOR® Agent application and the MLS Agent (Subscriber) Application.
Agent Update Form - To change personal information e.g. phone or fax numbers, mailing address, last name, etc., please use this form.
Agent Notification of Change Form - To affiliate, transfer or sever an agent from an office, please use this form.

Broker Forms

Broker REALTOR® Application Form and MLS Application - This form includes the SDAR REALTOR® Designated Broker application and the MLS Broker (Participant) Application.
Broker Authorization Form - This form allows designated brokers to authorize specific signing and charging privileges to specific individuals. A current copy of this form must be filed with SDAR to activate or ensure uninterrupted signing and charging privileges. This form must be filed or renewed at the beginning of every year or an interruption in service will occur.
Broker Notification of Change Form - To change your firm's information or status e.g. activate or inactivate MLS services, open or close a branch office or update office information, please use this form.
Designated REALTORS® Change Form - Designated REALTORS® Change Form.

SDAR Accounting Forms

All members can authorize auto-debits to pay their association dues and MLS fees. Designated REALTORS® with charging privileges can authorize auto-debits for these fees, as well as for other purchases such as classes, events and REALTOR® Store purchases.
